  her name is Kushina Uzumaki. She was the nine- tailes jinchurikki(the 9 tails was sealed inside her).
  During the birth of naruto, which was the time the 9 tails seal was the weakest, a masked man with the sharingan(not an uchiha/ not kakashi sensei) named tobi( an akatsuki from shippuden) used the sharingan to control the nine-tails to attack the village. the forth hokage(yondaime hokage/minato namikaze/naruto’s father)
  defeated tobi then sealed half the nine-tails chakra into naruto with his and kushina’s soul. Kushina will next time be seen when Naruto tries to extract the kyubi(9 tails) chakra in shippuden.
