  theres a resemblance betwen obito and shisui,and like sasuke,who found out that itachi was betwen the closest persons to him from a distance,maybe the mangekyo doesnt activate only at killing ur best friend by ur self,couse its the same thing for sasuke who regrets his bros death and reciving the mangekyo for that and for kakashi that regrets rins death (thats just my opinion) who probably died before he got his mangekyo and maybe when shisui found out that his older bro,obito(since kakashi is older than itachi,i assume that obito is older than shisui too) died he reacted like sasuke did and got his mangekyo,couse to me obitos mangekyo(now kakashis mangekyo), has probably the same level of jutsu as shisui,obito having best ninjutsu mangekyo in my opinion,its just kakashi wasnt made for it thats why we didnt see its full potentiall, and shisui having best genjutsu,their eyes actually have a single ability as well,so that can make me think their related even more,itachi got his when regretting shisuis death, thats why he got mad when he got accused for killing him back then,so i think shisui got his mangekyo like sasuke did(thats only my opinion
