  No a hokage hasnt got to die … the third hokage retired so minato could reach his goal and become the 4th hokage … when the 4th died the 3rd hokage stepped back in to fill the gap .. thats it …. but in the manga a lot hokages die and then anotherone gets named thats the normal procedure, so you could think that its just working with death. I think thats the only example where there are two hokages existing along together.
  
  And they still call Hiruzen third hokage cause there is no taking it back. He was called out third Hokage once and is keeping the title so the ninja still call him third hokage .. even if Minato was the “offical” Hokage then.
